This was an interesting episode because it goes through two perspectives, the one in Tony's head while he is in a coma, and the other one outside in the real world. The one in his head shows Tony if he were a regular working man who loses his ID disabling him from doing anything from booking a hotel, buying food, or leaving the conference he flew in Texas. He is battling with sinful acts (his gangster life and the choices that lead him to his coma), disease (his anxiety attacks, the dementia he was diagnosed with at the hospital within his coma dream), and has been dealing with a lot of death in the Family (old gangsters, rats who worked for the FDI to take down the Family, and friends who betrayed Tony). All these things, these choices Tony has made are finally catching up with him. He was shot by the man (Uncle Johnny) who Tony used as a prop-up boss, so when the things happen, the uncle would be arrested and not Tony and now his family is being watched by the press, getting all the attention dye to what happened in the previous episode. The Coma Tony acts nothing like the Tony we have seen throughout the show, he is more poised, calm, has manners, not an egoistic man just a regular working Joe. Another interesting thing about Coma Tony is he wants to give up on life after discovering he has dementia, just like Uncle Johnny.

This is where also see the men within the Family are acting, the siblings, and the wife. All these people are showing their true colors, but we have seen this before when the old boss died in the first season of the show. The difference is, we don't know who will be the boss for this Family when all the old characters have been killed by Tony. This could be the end of this Family, but we won't know until the end of the show. AJ is now in college but we find out he flunked college who has always struggled with academics from the moment he was introduced in the show, where Meadow flourished. Both kids were treated the same, yet they turned out with two different outcomes. This may be the beginning of AJ's journey of becoming a "gangster" filling in the shoes of Tony, like Tony did for his dad, I came to this conclusion when AJ was thinking of killing Uncle Johnny for accidentally shooting Tony. This near-death experience allowed us to see what would happen if the boss did actually die? No one cared as much about a death in the Family, than when everyone heard Tony was the next person in his death bed.
